1. Introduction
The following policy applies to all University staff
who wish to change their name on official University
records.
2. Process
2.1 When an employee wishes to change their
name on the official records of the University of Wollongong,
the following regulations apply,
2.2 The employee must provide documentary proof
of a change of name to Personnel Services at the University
of Wollongong, this may be effected by one of the following
methods:
2.2.1 Advertising the change of name in the
daily newspaper. The original date of the advertisement
must be sighted.
2.2.2 Provision a deed poll certificate,
2.2.3 Provision of a marriage certificate,
2.2.4 Provision of a birth certificate or passport.
These same regulations apply if an employee wishes
to revert to a former name.
3. Applicability
3.1 The change of name does not automatically
change an employees details for taxation purposes, as
the payment of salary is related to the tax file number.
If an employee wishes to change their name on the Taxation
records, they must contact the Australian Taxation Office.
3.2The change of name does not automatically
alter the name of a spouse or any children or siblings
who may also be employees of the University. If this
is to occur, it should be clearly specified in the documentary
evidence provided.
3.3 The change of name does not automatically
change the Superannuation records, the employee will
be required to separately contact their Superannuation
fund.
3.4 The change of name will not automatically
change the bank account name details of the employee.
The account name must be changed through the relevant
financial institution (bank, credit union, building
society).
