Financial Services - Insurance Claim Forms

All claim forms should be lodged with the Finance and Insurance Officer in Financial Services, together with all relevant supporting documentation eg. original purchase order, replacement quotation, witness statements, medical certificates, police reports etc.

You must notify the Finance and Insurance Officer as soon as you are aware of a situation that may lead to a claim. Late notification could lead to denial of the claim by the University Insurer.

Any loss or damage by theft should be reported to the Police and University Security.
