Who do I contact if I need help filling out the Access Form?

The Cognos Management Reporting Suite is made up of a series of reporting products. You need to fill out an access form to obtain access to the system. If you are unsure how to fill out the access form please contact a member of the Performance Indicator Project Team for assistance.

Important Note:

When completing an access form you will be asked to sign to say you have read, understood and accept the terms within the University's IT Acceptable Use Policy. Please read the policy before signing the forms.

The completed forms must be returned to the Performance Indicator Project Security Officer in building 113.
